Quality Nursing Care

The quality care we provide is accomplished by staff who are trained to care for the individual needs of each
resident. We provide care according to each resident's particular functioning level, ranging from early
dementia to rehabilitation and medically complex needs.

Care of the resident is directed by an attending physician. Each local clinic has specific physicians who
serve Bethany Residents. Residents may retain their own community physicians if they prefer to travel to
the clinic for follow-up appointments.

A Licensed Registered Dietitian conducts ongoing assessments of each resident's
nutritional status. Specialized diet plans are provided as appropriate.

North Dakota nursing home residents are classified into one of 34 levels of care on a quarterly basis. This
classification is based on the intensity of services received, which determines daily rates by facility. (For more
information regarding North Dakota rates, go to www.health.state.nd.us and select the topic, "Nursing Facility Charges.")

Bethany's skilled nursing facility is certified for Medicare and Medicaid (Medical Assistance).

Social Services

The Social Services Department facilitates admissions to Bethany Homes, identifies the psychosocial
needs of the residents, assists in helping residents achieve their highest potential and facilitates
discharge planning. Social Services staff work with all departments to develop a care plan to assist
residents in achieving their goals, which often include returning to their home or lesser level of care
following rehabilitation.

Resident Rights, including advanced directives, are also a priority of the Social Services Department.

Rehabilitation Services

Licensed therapists provide physical, speech and occupational therapy for individuals recovering
from major illness and surgery, neurological conditions, and orthopedic conditions. The therapist
evaluates the individual and determines a plan of care, including frequency
and intensity, that best meets the resident's abilities and goals.

Spiritual Care

Our full-time Chaplain, Rev. Phil Larson, provides pastoral care at Bethany. His services include
personal visitations and availability for counseling for residents and their families. Our Chaplain
encourages spiritual vitality and helps bring hope and comfort. Worship services are held regularly
on each floor as well as in the Mavis Whiting Chapel. Additionally, area churches representing a
variety of denominations hold regular services at Bethany.

Admissions/Waiting List

The first step in the process of admitting to Bethany CARES, is to complete an Application for Admission*
form. When this completed application is received, that person is placed on the waiting list. The average
wait for admission varies; therefore it is helpful to plan ahead.

Individuals on the waiting list will be contacted in chronological order. If there is interest in the opening, an
assessment will follow. Once a room is offered, the move must take place within two days.

When a room is offered and has been accepted, the next step is securing an appointment with a physician to
complete a "History and Physical" examination (must be current within 5 days). Additionally, orders for
admission, medications and treatments are required. Our Admission Coordinator will assist you in this process.

Upon admission to Bethany, a multidisciplinary team meets with the new resident and family to gather information
necessary to provide appropriate care, as well as to discuss goals regarding future plans. These potential plans
include a long-term stay, returning home, home health care, assisted living, basic care, etc. Our social service
staff will maintain involvement at all times.
